MohsLabServices.com is a website of Mohs Pros, Inc. ("Mohs Pros", "we"), operated on our behalf by HeyWaiter Inc.. This policy explains what personally identifiable information we collect through the site and what we do with it.
What we collect
- What you send us through the request form: your name, role, practice name, email, phone, the city, state and ZIP code of your lab, the services you're interested in, your surgery schedule, start date, current setup and any message.
- Technical data our hosting provider processes to deliver and protect the site, such as IP address, browser type and pages requested.
Please don't send patient or health information through this site. The form is for information about your practice and lab only.
How we use it
- To respond to your request, contact you and prepare a quote
- To decide where Mohs Pros can offer service next, including keeping waitlist requests by state
- To keep the site secure and working
Who we share it with
We don't sell your information or share it for advertising. We share it only with service providers that help us run the site and handle your request (for example, our hosting and email providers, including Cloudflare), withHeyWaiter Inc. as the site operator, and when the law requires.
Cookies and tracking
The site doesn't use advertising cookies. If we add analytics, we'll describe it here. Our security check on the form (Cloudflare Turnstile), if enabled, may process data needed to tell people from bots.
Do Not Track: the site doesn't change its behavior in response to browser "Do Not Track" signals, because it doesn't track you across other websites. We don't allow third parties to collect information about your activity across sites through this site.
Reviewing and changing your information
You can ask us to see, correct or delete what you've sent us by emailing hello@mohslabservices.com. Some records we must keep by law.
